Visiting Us

To prepare for your child's visit, we offer some information for you to review, as well as some forms for you to complete prior to coming in.

Please complete and bring with you to your visit:

1. Patient Information - Please fill in all areas, sign and date. We will also make a copy of your insurance card when you come in, so please remember to have this with you at the time of your visit. If your insurance requires you to to have a referral form completed by your primary clinic, please bring a copy of the referral with you. We will also collect a copay (if applicable) at the time of your visit.
   
   
2. Health History Form - Please complete this form and bring with you to your child's visit.
   
   
3. X-rays - If your child is going to be seen by our urology section and x-rays have been ordered, please read the "Dear Parents" letter and bring your child's films with you to your visit.
